Immunization
Requirements
Welcome to NDSU, a campus that strives to create an environment which
supports student health and wellness. As a condition of enrollment,
the State Board of Higher Education requires students to provide a
record of two (2) doses of Measles, Mumps and Rubella Vaccine. (Sometimes
noted as MMR).
NOTE:
Do not send vaccination records or copies of records as they will
not be stored, nor will they be available for transfer. The proof
of vaccination must be recorded on the reverse side of this form by
your health care provider.
Students who have been admitted to NDSU are asked to submit their
vaccination information prior to their first registration. You may
obtain the form at the Student Health Service office and turn it in
at orientation.
A hold may be placed on your registration attempt for any of the
following reasons:
- You have not submitted any record of MMR vaccination.
- You need a second dose of MMR prior to registration.
(Wait 31 days between doses).
- You received a dose of MMR before age one (1).
- You had your vaccination prior to 1970.
- The proof sent was not signed by a physician/nurse/public
health representative or was not an official school record.
- Immunity for Rubella (German Measles) must be proven by laboratory
titer in the absence of vaccination.
Exception for this requirements are granted when:
- Documented medical conditions such as a febrile illness, pregnancy,
or certain allergies exist.
- The second MMR will be received within one month following
enrollment.
- Religious beliefs preclude participation in a vaccination
program.

Federal law prohibits us from requesting medical information for you.
