Applying for Accommodations
The student is responsible for initiating a request for accommodations by making an appointment with a staff member in Disability Services (DS) at least thirty days prior to the term of enrollment. In order to receive accommodations, the student is required to provide current documentation of the disability as well as the need for the reasonable accommodation. In requesting accommodations that require time to prepare, such as recording textbooks onto tape or hiring sign language interpreters, requests need to be made early enough to allow ample time to fulfill the request, preferably 60 days before the beginning of the semester.
The student meets with a DS staff member to do the following:
• provide initial information about him/herself (complete an intake)
• review documentation
• discuss needed accommodations
• go over policies and procedures
• complete appropriate accommodation forms
The DS staff member will provide letters to instructors verifying a disability, and stating the reasonable and appropriate accommodations recommended by Disability Services as needed. The student is responsible for delivering the letter to the instructor during the instructor’s office hours as soon as possible at the beginning of the semester.
