Residence Life Request procedures based on medical, psychological, or disability-related needs:

NDSU student living facilities and dining services are designed to extend the student’s educational experience beyond the classroom. The learning and residential environments at NDSU provide students with an opportunity to enhance a variety of educational, cultural, social, and recreational activities.

The request for residence life medical, psychological, or disability-related need is considered carefully.

To assist with this process, a student should provide a written request, including the following items:

A.  A clear description of the desired residence life need;

B.  An explanation of why this request is necessary and how it relates to the impact of the condition;

C.  A statement about how the student will manage if the request cannot be met.

D.  The name, address, phone and fax number of the provider who has diagnosed and/or managed the condition.

NOTE: This is necessary to obtain current documentation of the condition that is the basis for the request.

E.  A completed and signed release of information for us to send to your healthcare provider with the request for information.
