Non-Credit Forms
The forms on this page are for use by organizations, businesses, and/or individuals who plan to offer a non-credit workshop or conference. Workshops and conferences may be offered with or without Continuing Education Units attached to them. A Continuing Education Unit (CEU) is used for activities that do not qualify for college credit. The CEU may be used for the measurement, recording, transfer and recognition of participation by adults in non-credit activities.
A CEU is defined as "ten contact hours of participation in an organized continuing education experience under responsible sponsorship, capable direction and qualified instruction."
Workshop/Seminar/Conference Proposal Form
Use this form to propose a workshop, seminar, or conference that will not offer CEUs.
CEU Proposal Form
Use this form to propose an workshop, event or class that you would like to offer CEUs for.
Cancel Registration Form
Use this form to cancel your registration for a non-credit conference, workshop or class.
Note: A $20 processing fee will be assessed per person for cancellations received after the start of the course.
