NORTH DAKOTA STATE UNIVERSITY
RESIDENCE LIFE REQUESTS
(Based on Medical, Psychological, or Disability Related Needs)
NDSU student living facilities are designed to extend the student's educational experience beyond the classroom.
The learning and residential environments at NDSU provide students with an opportunity to enhance a variety of educational, cultural, social, and recreational activities. Residence Life offers a wide range of living environments, capable of catering to almost any need that a student may have.
In order to fully consider a Residence Life request based on a medical, psychological, or disability-related need, THE STUDENT will need to provide a written request to the Disability Service Director with the following items included:
A. A statement about the student's condition and how living in a
Residence Life environment will impact the condition or the
treatment of the condition.
B. A statement with suggestions that may minimize the impact of
the condition in a university living environment.
C. Contact information for the licensed professional who diagnosed
or is treating the condition. Include name, address, phone, and
fax number. The student will need to include a signed release
for this individual to exchange information with NDSU Disability
Services. Releases can be obtained through the licensed
professional's office.
D. A signed FERPA release when a family member needs to be involved
in the request. Below is a link where the release can be found.

The written request, along with the completed and signed releases, should be sent to: Disability Services, Dept. #5160, PO Box 6050, Fargo, ND 58l08-6050 or faxed to 701-231-5205. Information submitted is treated as confidential.
NDSU Disability Services will contact the licensed professional to request the necessary documentation. Current documentation is helpful to the request.
