Implementation of Pre-Employment Criminal Record Disclosure

NDSU Policy 112 on pre-employment criminal record disclosure became effective for all individuals hired after July 1, 2002, including regular employees, temporary/part-tim employees and student employees. The Criminal Record Disclosure form is available and may be completed on-line at http://www.ndsu.nodak.edu/forms/.

The following procedures will be used to incorporate this requirement into the hiring process:

1. For all regular employees:

- A completed and signed Criminal Disclosure form must accompany the Request to Offer or be provided at the tine authorization to offer a broadbanded position is requested through the Office of Human Resources/Payroll.

- If an individual accepts the offer, the completed form will be filed in the individual's official personnel file (see Policy 718 - Public/Open Records). *Please note that the hiring department is responsible for assuring that forms for facultu, lecturers, and non-broadbanded Extension and Agricultural Experiment Station staff are filed in the relevant Dean's Office.

The forms for all other employees will be filed in the Office of Human Resources/Payroll.

- Departments may choose to ask all interviewees to complete the Criminal Record Disclosure form in order to facilitate additional offers if they are necessary. Forms for those not actually hired should be filed with the application materials and treated as part of the position transaction file.

2. For all graduate assistant and temporary, non-benefitted and timeslip employees, including all student employees:

- A completed and signed Criminal Record Disclosure form must be submitted with Form 100/102.

Additional information:
- Fax copies of the completed/signed form are acceptable.
