Examination will not be accepted. Your
exam must be schedule with the Graduate School.
Must be submitted to the Graduate School with the first draft of
the thesis or paper.

- This form must be printed on light green paper
- This form should be filed after the final portion of the
preliminary exam
- A negative vote by more than one member of the student's
committee will signify failure of the examination.
- This form must be printed on goldenrod paper.
- At the close of the oral examination, each examining
committee member shall record in writing his/her approval or
disapproval of the candidate. This form must be filed with the
Graduate School within seven (7) days of the exam.

Approval Page
The Approval Page is bound into each copy of the
disquisition. It must be printed on 20lb. acid-free
paper.
