Non-Benefitted Hire Process

Includes timeslip students and non-students, graduate students and part time academic staff.

If hiring a new employee or a rehiring an employee who had NOT been actively employed within the last 12 months:

Required forms include:

  1. Hiring Form 100
  2. Employee Data Form
  3. Criminal Disclosure Form
  4. Annual Notice of Policies & Workforce Safety & Insurance Notice
  5. W-4: Employee Withholding
  6. I-9: Employee Eligibility Verification
  7. Direct Deposit Form
  8. NDWR: MN residents withhold MN tax

If rehiring a student/temporary employee who had been actively employed within the last 12 months:

Required forms include:

  1. Hiring Form 100
  2. Criminal Disclosure Form
  3. Annual Notice of Policies & Workforce Safety & Insurance Notice

 

 

Early Employee ID Process

The Office of Human Resources/Payroll has a practice in place to issue an employee ID number to new hires prior to their start date.  We will create an employee ID number early to help facilitate the process for new hires (i.e. getting an email account, keys, parking permits, etc.)  In order to request the employee ID number, an Early Employee ID Request form has been created.  The hiring department should complete this form, attach a copy of the signed Request to Offer and the completed Employee Data form that the employee has filled out.
