Separation Information
Staff: An appropriate length of notice is at least two (2) weeks. Please refer to Policy 181.
Faculty: A faculty member may terminate an appointment effective at the end of the term of the appointment by giving notice in writing at the earliest possible opportunity, but not later than May 15, or one month after receiving notification by the institution of the terms of an appointment for the coming academic year, whichever date occurs later. Please refer to Policy 350.3, section 5.

Health and dental insurance will continue through the last day of the month following the month in which you separate employment.
In addition to this, you are entitled to continue health and/or dental insurance in NDPERS group plan for up to 18 additional months. You would be responsible for the full premium plus a 2% NDPERS administrative charge. Information regarding the cost of continuing the insurance will be provided either during the exit interview or mailed to the home address on file.
NDPERS life insurance will continue through the last day of the month following the month in which you separate employment.
Tri-Term Insurance through ReliaStar may be converted to an individual plan or it too will be discontinued on the last day of the month following the month in which you resign. A postcard may be picked up in Human Resources to notify ReliaStar of your decision.
If you have money remaining in a Health Care or Dependent Care Spending Account you may submit claims for that money until February 28 of the following year in which you resign. However, claims for the Health Care Account must be for expenses incurred prior to your separation date.
If you participate in the Health Care Account, you have the right, by Federal Law, to continue making contributions to that account on an after-tax basis. Please contact Human Resources for details regarding this contribution.
After your separation, you will not be able to contribute to your TDA. However, your account will continue to accrue interest and you should receive periodic updates from your TDA Company. Contact your TDA Company representative for further information.
Group coverage will terminate on the last date of employment.
NDPERS: Upon separation of employment, you will be given information regarding your options, which include: refund, direct rollover or later withdrawal of your account. You will continue to receive your annual statement if you elect for later withdrawal.
TIAA-CREF: Upon termination of employment, contact them directly at 1-800-842-2776, to discuss your options. You will continue to receive your quarterly statements.
Social Security benefits can begin as early as age 62; however, the benefits will be reduced accordingly, full benefits currently begin at age 65. If you file for social security, you must do so 3 months prior to receiving your first check. When filing a claim, your social security number, proof of age, last W-2 form and/or last tax return must be brought for all people involved in the claim. Contact the Social Security Office, Federal Building, Third Floor, Fargo, or call 1-800-234-5772, for further information.
You will receive 100% pay for any annual leave hours accumulated up to your separation date.
Sick leave is paid out at a rate of 10%, if you have been employed for 10 continuous years or more.
If receiving a payment for annual/sick leave, see Karin (231-8990) in Payroll for any tax questions/concerns.
All W-2 forms are mailed to the home address we have on file as of January 31st.
Please notify Human Resources or Payroll of any address changes prior to January 1st.
All payments to the United Way Fund will stop with your last paycheck. There is no obligation for NDSU or you to continue or fulfill this commitment. It is your choice to continue this commitment directly with the United Way. Their address is United Way, Box 24, Fargo ND 58102.
All payments to the NDSU Alumni/Development Foundation will stop with your last paycheck. It is your choice to continue this commitment directly with NDSU Alumni/Development Foundation. Their phone number is 231-6800.
Balance must be paid in full. Please make payments to the Computer Store in the Varsity Mart, questions may be addressed to 231-8013.
If you have participated in the Credit Union, you may remain a member after separation of employment. However, all activities are conducted through Northland Educators Credit Union at 1404 12th Avenue North, Fargo ND 58102 or (701) 293-6132.
Turn in all keys, calling cards to your supervisor by your last day of employment.
