Engineering GPA Calculation WorksheetAcademicThe professional program engineering GPA calculation worksheet is a list of courses which must be completed before being fully accepted into the program. The upper division courses are restricted to students who have been admitted. A 2.8 engineering grade point average and a C or higher in all courses are also application requirements. FERPA FormAcademicAll students requesting Mechanical Engineering Faculty to serve as a reference for scholarships, job searches, co-ops or any other such purpose, need to have a FERPA from on file in the ME Office. Download Form
Pre-Order Form, Required for ALL PurchasesAccountingAll individuals (students/faculty/staff) purchasing items through the Mechanical Engineering Department MUST have this form on file before doing so. You may also need to check in the ME Office for specifics about ordering with certain companies. If ordering for a Senior Design project, a yellow budget form is also required. Without this form and a budget on file, you do not have proper authorization to purchase items for any project. Download Form

Reimbursement Form - SuppliesAccountingIn order to be reimbursed for expenses not charged directly to the Mechanical Engineering Department, a reimbursement form can be filed. You will be reimbursed ONLY if this form is completed and approved PRIOR to purchases. NDSU does not pay sales tax, therefore, this may not be reimbursed. An ORIGINAL receipt must also accompany this form.Download Form
Reimbursement Form - TravelAccountingIn order to be reimbursed for approved travel expenses not charged directly to the Mechanical Engineering Department, a travel reimbursement form can be filed. ORIGINAL ITEMIZED receipts must also accompany this form for all items needing reimbursement, and credit card/bank statements may be needed as supporting documentation.Download Form
Scholarship Application 2014AcademicCollege of Engineering General Scholarship Application used to award all scholarships available through the ME Department. All current Mechanical Engineering students are eligible to apply. Form is DUE on February 21st, 2014 to the Mechanical Engineering office in Dolve 111. Scholarship awards will be applied to the Fall 2014 semester.Download Form

Time Sheet First Half of MonthAccountingStudents working in the Mechanical Engineering Department may print a time sheet for the first half of the month (1 - 15) using the link below. The time sheet is due in the ME Office within a few days after the 15th and needs to be signed by both you and your supervisor. Download Form
Time Sheet Second Half of MonthAccountingStudents working in the Mechanical Engineering Department may print a time sheet for the second half of the month (16 - 31) using the link below. The time sheet is due in the ME Office within a few days after the end of the month and needs to be signed by both you and your supervisor. Download Form
Trial CardAcademicA trial card is used as an aid for planning a class schedule. A signed trial card is needed when permits are required for courses. Trial cards can be found in the Mechanical Engineering Department Office, or can be printed from the web site.Download Form
