Application for Undergraduate/Professional Degree

INSTRUCTIONS:

  • Purpose: Students plan their educational pursuits differently and may opt to complete more than one degree type, or more than one major or minor. All candidates for a baccalaureate/professional degree must declare their intent to graduate so that the University acknowledges and records all degrees, program majors and minors on the final academic record. If students fail to complete the required courses during the semester they have indicated, they must reapply by submitting a new Application for Undergraduate/Professional Degree.

  • Deadlines:
    • If you are graduating Fall Semester, submit this form no later than the 3rd week of September
    • If you are graduating Spring Semester, submit this form no later than the 1st week of February
    • If you are graduating Summer Semester, submit this form no later than the 2nd week of June
    • Please see Academic Dates & Deadlines for specific deadline dates for each semester

  • Graduation eligibility is determined by the College Liaison in the Office of Registration and Records

  • This degree application is independent of Commencement Ceremony Participation (see post-submission "Thank You" page for more information)

NOTE: If you are earning two different degree types (i.e., a "Bachelor of Arts" for one major and a "Bachelor of Science" in another), please complete and submit another application for the second degree.
