Application for VA Education Benefits (22-1990)

This is completed to apply for Chapter 30, 1606 and 1607 VA benefits and must be sent with supporting documents specific to each chapter.

Change of Program/Place of Training (22-1995)

This is completed by Chapter 30, 1606 and 1607 students who change their major or their school. It is also completed by students who are applying to transition from 1606 to 1607 and is submitted with the supporting documents needed for application to Chapter 1607.

Change of Program/Place of Training (22-5495)

This is completed by Chapter 35 students who change their major or their school.

Notice of Basic Eligibility (NOBE)

This must be submitted with applications for Chapter 1606 and 1607.

Discharge from Active Duty (DD-214)

Copy 4 must be submitted with applications for Chapters 30 and 1607.

Certificate of Eligibility (COE)

This is provided to the student from VA and indicates their apporved eligibility for VA benefits.

Kicker Contract

This will be provided to the student from their unit, if applicable, and will be submitted with an application for benefits.
