IT Team Nomination Form
The Division’s IT Team Award recognizes a group of individuals who have established a well-defined goal or activity and successfully work together to achieve results that have significant impact on the campus community.
Provide examples to exemplify an award-winning team as it relates to the following:
- Achievement and success driven: Describe the team’s goal or objective/project, noting planning, implementation, conflict-resolution ability, adaptability and flexibility, and outcome.
- Communication: Does the team clearly and accurately communicated with stakeholders and with each other. Is the team customer-focused? Does the team exhibit a shared trust, open and honest discussions and respect constructive feedback?
- Commitment & Leadership: Does the team show passion and dedication to the goal? Does the team have a core leader to guide, make decisions and keep the team moving forward
