IT Service Requests

To request a new IT Service, please download and complete the IT Service Request form. Once you have completed the form, you can submit it in one of the following ways:

1. E-mail the form to NDSU-IT-REQ@LISTSERV.NODAK.EDU, or
2. Print the form and send it via campus mail to the Office of the VP for IT, IACC 204
