Disability Documentation Guidelines
If you are a student with a condition that impairs a major life activity and plan to attend NDSU, here are some guidelines for accessing reasonable accommodations. It's the student’s responsibility to begin this process.
In order to expedite the provision of services, we encourage you to contact the Disability Services office at least 60 days prior to the semester in which you plan to enroll.

This documentation can be an assessment or a report from an appropriate professional such as a physician, rehabilitation counselor, psychologist, neuropsychologist, or audiologist. This documentation should state your disability/diagnosis with functional limitations. Recommendations for academic accommodations and support services will be considered based on the specific limitations of the disability.
NDSU adheres to the Guidelines for Disability Documentation adopted by the North Dakota Disability Services Council.
If you plan to live on campus and have special living requirements, please contact the Department of Residence Life (Housing) at 701-231-7557 or 800-572-8840 to submit your housing contract and deposit. If you're requesting a specific room configuration based on disability, please follow these procedures.
To inquire about financial aid, please contact the NDSU Student Financial Services office in person at Ceres Hall 202 or by phone at 701-231-6200 or 800-726-3188 to obtain a financial aid application. Disability Services does not provide financial assistance.
Rehabilitation Consulting & Services is an employment focused program to help eligible students with disabilities either obtain or maintain employment that matches their capabilities.
