Dental Insurance

There are two options available for dental insurance.  Both plans are employee-paid and taken out via payroll deduction.

Delta Dental

The premiums below are in effect January 1, 2015 through December 31, 2015.

  • Dental plan annual maximum benefit per person:  $1,000
  • Orthodontia lifetime maximum benefit per person:  $1,500
  • The deductible includes total expenditures per person for all basic and major treatment combined. 

Preventive and diagnostic treatment
(oral exams, cleanings, x-rays)


Basic Restorative Care
(fillings, oral surgery, periodontics)

Major Restorative Care
(crowns, bridges, dentures)
