Application for Undergraduate/Professional Degree

Students plan their educational pursuits differently and may opt to complete more than one degree type, or more than one major or minor. All candidates for a baccalaureate/professional degree must declare their intent to graduate so that the University acknowledges and records all degrees, program majors and minors on the final academic record. Degree eligibility is determined by the College Liaison in the Office of Registration and Records.

This "Application for Degree" (declaring that all graduation requirements will be met) is independent of "Commencement Ceremony Participation" (walking in the graduation ceremony). See the "Thank You" page that appears immediately after submitting this form for more information about the Commencement ceremony and your diploma.

If a student fails to complete the required courses during the semester they have indicated, they must reapply by submitting a new Application for Undergraduate/Professional Degree under a new expected term of degree completion.
