PLEASE NOTE: This form is intended for Undergraduate students. Returning Graduate students should contact the Graduate School for more information. Continuing Education students in non-degree coursework should contact Distance and Continuing Education.

Deadlines for Submission:
  • Returning from an NDSU Academic Suspension? You must submit your petition 30 days prior to the start of the term for which you seek readmission.
  • For all other submissions, it is recommended you allow 5-7 business days prior to the start of the term for which you seek readmission.

Please be aware that according to NDSU academic policy, each baccalaureate degree that a student earns must be different. Financial aid eligibility may be impacted if the degree types are not different and some programs are only offered under one degree type. For instance, if a student has already earned a B.S. degree, he may seek a B.A. or a specialized degree, such as a B.S.N. or B.F.A. but he cannot earned another B.S. degree with a different major. For more information, see the Second Degree policy in the Undergraduate Bulletin.

You must arrange for official transcripts to be sent to: Office of Registration and Records, NDSU Dept. 5210, P.O. Box 6050, Fargo, ND 58108-6050. Failure to list all colleges, universities, online courses, and schools previously attended may result in denial of admission, recission of admission, dismissal, loss of credit(s), or other appropriate sanctions. In accordance with NDSU Policy 713: Records Retention, some student records are purged after a period of five years of non-attendance. Students who have not attended NDSU for five or more years will be required to submit (or re-submit) transcripts for all coursework previously completed at other institutions.

PLEASE NOTE: NDSU honors suspensions of other institutions. Further, students suspended from NDSU or any other institution may not transfer coursework into NDSU that was completed during the suspension period unless prior approval has been granted through an appeal process. Transfer and returning students who fail to report all previous college work are subject to dismissal or loss of credit or both. Courses previously completed at NDSU may only be repeated at NDSU, with the exception of Tri-College courses.

All students making application to North Dakota State University MUST ANSWER THE FOLLOWING QUESTIONS. An affirmative response to any of the questions will not automatically prevent admission, but you will be asked to provide additional information. The information will be reviewed by a campus committee. Falsification or omission of information may result in a denial of admission, recission of admission, dismissal or other appropriate sanctions.

"Dismissed for disciplinary reasons" means a permanent separation from an institution due to conduct or behavior. "Suspension for disciplinary reasons" means a sanction imposed for disciplinary reasons that results in a student leaving school for a fixed period, but not permanently.

Please be aware that any additional documentation required must be received at least 30 days prior to the start of the term of which you are seeking re-admission. Failure to provide accurate and truthful information may result in denial, recission of admission, dismissal, loss of credit, or other appropriate sanctions.
