APPEAL FORM AVAILABIITY AND DEADLINES: 

Financial Aid Disqualification Appeal Form

Submit the Financial Aid Disqualification Appeal Form below if the Financial Aid Disqualification is due to any of the following reasons:

  • Grade Point Average (GPA) less than 2.0
  • Completion of Attempted Credits less than 67%
  • All Failing Grades

Maximum Attempted Credits/Time Frame Appeal

Complete the Maximum Attempted Credits/Time Frame Appeal form if either status below applies: 

  • Financial Aid Disqualification due to exceeding the maximum attempted credits/time frame limit
  • Financial Aid INFO status for nearing the maximum attempted credits/time frame limit

If the student is appealing Maximum Attempted Credits/Time Frame or Financial Aid INFO Status in addition to another disqualifying item such as GPA or 67% completion of credits, both the Maximum Attempted Credits/Time Frame Appeal AND the Financial Aid Disqualification Appeal Form must be submitted.

NOTE: Students appealing an INFO status have until the end of the semester to submit their appeal.

Not sure which form to use?

  1. If the student is disqualified due to one or more of the reasons below, he or she must submit the Financial Aid Disqualification Appeal Form:
    • Grade Point Average (GPA) below 2.0
    • Completion of Attempted Credits less than 67%
    • All Failing Grades
  2. If the student is disqualified for exceeding the Maximum Attempted Credits/Timeframe or due to a Financial Aid INFO status, he or she must submit the Maximum Attempted Credits/Time Frame Appeal Form.

  3. If the student is disqualified due to both 1 and 2, both appeal forms must be submitted.
