Please note the list below contains all verification forms.  Complete only the required forms listed on your Campus Connection "To Do List".

How to Request a Tax Return Transcript from the IRS

If you are NOT eligible to use the IRS Data Retrieval Tool, you and your spouse (if you are an independent student and married as of today) or you and your parent(s) whose income you used on the 2017-18 FAFSA must submit a Tax Return Transcript.  Please note that a copy of your Federal IRS Tax Returns or an account transcript are NOT acceptable.

There are a variety of ways to request a Tax Return Transcript:

Online

  • Visit https://www.irs.gov/Individuals/Get-Transcript
  • Click “Get Transcript by mail” to order your Transcript
  • Enter requested information exactly as it appears on your tax return and click “Continue.”  If filed a joint return, use the SSN of the person listed first on your taxes. 
  • Select “Return Transcript” and the correct year. We are unable to use and Account Transcript for Verification purposes.
  • Click Continue.
  • Your transcript will be mailed within 5 to 10 business days.

Postal Mail

  • Print and fill out the form 4506-T. The form can be found at https://www.irs.gov/pub/irs-pdf/f4506t.pdf
  • Mail or fax the completed form to the corresponding address found on the 2nd page.
  • Note - This form allows you to request duplicate W-2's, Tax Return Transcript or Letter of Non-Filing

Appointment with the IRS

  • Based off your state.
  • Gives you the option to press 1 if you would like to make an appointment.
  • Average wait time is 7-10 minutes.

Requesting an IRS Verification of Non-Filing Letter

If you and/or your spouse (if you are an independent student and married as of today) or you and/or your parent(s) whose income you used on the 2017-18 FAFSA did not file a 2015 Federal Tax Return, you are required to obtain documentation from the IRS that you did not file. An IRS Verification of Non-Filing Letter provides proof that the IRS has no record of a filed Form 1040, 1040A or 1040EZ for the year you requested.

Follow these instructions on how to request an IRS Verification of Non-Filing Letter

Non Tax filers can request an IRS Verification of Non-filing of their 2015 tax return status, free of charge, from the IRS in one of two ways:

Paper Request Form – IRS Form 4506-T

Download IRS Form 4506-T at https://www.irs.gov/pub/irs-pdf/f4506t.pdf 

  • Complete lines 1 – 4, following the instructions on page 2 of the form.
  • Line 3: enter the non-filer's street address and zip or postal code. Use the address currently on file with the IRSLine 5 provides non-filers with the option to have their IRS Verification of Non-Filing Letter mailed directly to a third party by the IRS. Do not have your IRS Verification of Non-Filing Letter sent directly to NDSU.
  • Line 7: Select the checkbox on the right hand side for Verification of Non-Filing.
  • Line 9: Year or period requested field, enter "12/31/2015".
  • The non-filer must sign and date the form and enter their telephone number. Only one signature is required when requesting a joint IRS Verification of Non-Filing Letter.
  • Mail or fax the completed IRS Form 4506-T to the address (or FAX number) provided on page 2 of Form 4506-T.
  • If the 4506-T information is successfully validated, tax filers can expect to receive a paper IRS Verification of Non-Filing Letter at the address provided on their request within 5 to 10 days.
  • Sign and submit the IRS Verification of Non-Filing Letter to NDSU; make sure to include the student’s name and ID on the letter.

Telephone Request

Available from the IRS by calling 1-800-908-9946

  • Tax filers must follow prompts to enter their social security number and the numbers in their street address. Generally, this will be numbers of the street address that was listed on the latest tax return filed. (Problems entering your street address?)
  • Select "Option 2" to request an IRS Verification of Non-Filing Letter and then enter "2015".
  • If successfully validated, non-filers can expect to receive a paper IRS Verification of Non-Filing Letter at the address provided in their telephone request within 5 to 10 days from the time of the request.
  • IRS Verification of Non-Filing Letter requested by telephone cannot be sent directly to a third party by the IRS.
  • Sign and submit the IRS Verification of Non-Filing Letter to NDSU; make sure to include the student’s name and ID on the letter.

W2 Information

A W2 statement is a standard tax form showing the total wages paid to an employee and the taxes withheld during the calendar year. It is prepared by an employer for each employee.

Where can I obtain a copy?

A copy of your W2 form can be obtained from your employer.

Please note that W2 statements can come in many shapes, sizes and colors but they will all have the same basic information however.

Submitting Verification Forms

There are a variety of ways you can submit your verification information.  Please type the information into the form, print, and sign them prior to submitting, any unsigned forms will be returned to you for signature.  You may submit the forms in the following ways:
