Submitting Verification Forms


There are a variety of ways you can submit your verification information. Type the information into the form, print, and sign prior to submitting; any unsigned forms will be returned to you for signature. You may submit the forms in the following ways:

DocuSign

The only DocuSign forms available are the 2020-2021 Dependent or Independent Verification Worksheets. DocuSign forms should not be submitted using the Verification Document Upload Form as explained below.

Scan or Photograph and submit online:

Print, sign and scan all documents; then go to www.ndsu.edu/onestop/forms/verification/ to complete the Verification Document Upload Form, attach your documents and submit them. When the form opens it will require you to enter your student ID number. Then click on the Populate Student Info button.

Once you click the Populate Student Info button, the Authentication Required box will pop up. This requires the student to log into the form with their Campus Connection username and password to ensure the student ID number matches the correct student. Once you have entered your username and password click the Submit button.

NOTE - You must complete the NDSU New Student Setup to activate your Username, or you will be unable to log into the form. Follow the step-by-step instructions to activate your Campus Connection account. The normal format for this ID is firstname.lastname. If you are unsure whether or not you have activated your account check through the Have I Claimed My Account page.

If you have previously attended another North Dakota University System (NDUS) institution, you may already have an existing Campus Connection account. Please continue using this existing account username and password at NDSU.

If you experience problems or have questions about your System ID, please call the North Dakota University System Help Desk at 1-866-457-6387 or submit a Help Desk ticket at helpdesk.ndus.edu 

After logging in, the Student ID, name, email address and phone number will have been added at the top of the form. In the Documentation Details section of the form select the year the verification documents are for along with who will be responsible for uploading the necessary documents. This form allows for the student to attach files as well as the form to be routed to a parent/guardian to attach files; select the option that best matches the situation. If the student is routing the form to a parent/guardian to attach documents, fill in the Parent/Guardian Name, Parent/Guardian Email and Confirm Parent/Guardian Email fields.

If the student is attaching files, click the Attach Files button and the Attachments box will open.

Click the Browse File button to browse for the file then click the Add button. Files must be added one at a time. Once all files have been attached by the student, click Close.

Click the check box certifying the student has completed their portion of the form and click Submit.

The Thank You message will pop up. Read through the included information.  

Click the Close button in the toolbar to close the form.

If the form is being routed to a parent/guardian, they will receive an email with a link to the form. It will look like the image below with a blue link that reads "Click here to upload verification documents". Click that link to begin uploading parent/guardian documents.

In the attach documents section of the form, click the Attach Files button and the Attachments box will open.  If the student has already attached files, there will be a number next to the Attached Files indicating how many documents have already been attached.

Click the Choose File button to browse for the file then click the Add button. Files must be added one at a time. Once all files have been attached by the parent/guardian, click Close.

Click the check box certifying the parent/guardian has completed their portion of the form and click Submit.

The Thank You message will pop up. Read through the included information.  

Click the Close button in the toolbar to close the form.
