NDSU Faculty/Staff Membership
Faculty or Staff who are currently employed by NDSU may sign up for a Wellness Center membership, by visiting the Customer Service Desk at the Wellness Center to obtain the proper forms. All Faculty/Staff members must complete:
- a New Member Form
- a Standard Liability Waiver
- provide their NDSU Bison ID Card
- benefited faculty and staff should also complete a Payroll Deduction Form (see Payment Options below for additional information).
These forms need to be processed at the Wellness Center Customer Service Desk to activate a new membership.
All required forms MUST be signed and completed before the member will be allowed to utilize the Wellness Center activity areas. If the Payroll Deduction method of payment is chosen, the Faculty/Staff membership will stay active until a Payroll Cancellation Request Form has been submitted. In order to keep our records current, Faculty/Staff memberships on Payroll Deduction are required to go through a brief annual renewal process that requires each member fill out a Standard Liability Waiver.
Faulty/Staff Memberships are eligible to sponsor Guest(s) and (1) Dual Member.
