Fargo Cass Public Health will provide the following immunizations for NDSU employees:
- Flu*
- COVID-19*
- HPV*
- Tdap (tetanus)*
- Hepatitis A
- Hepatitis B
- Pneumonia*
- MMR (measles, mumps, rubella)*
- Shingles
- RSV
Please bring health insurance or Medicare information to the clinic.
*Employees without health insurance can receive these vaccines for low to no cost.

Interested employees should complete the consent form and screening checklist below and either email them to CWavrin@FargoND.gov or bring it with them to the clinic. Alternatively, individuals can arrive as a walk-in and complete the paperwork onsite.
