Accident Report Process for All State Fleet Users
When a State Fleet vehicle is in a collision or any type of damage occurs to the vehicle, the driver of that State Fleet vehicle is responsible for following the OMB-Risk Management Vehicle Crash/Damage Procedures and completing the NDSU Motor Vehicle Incident Reporting form. Driver should fill out the accident report form, found in the motor pool vehicle packet or at the NDSU Motorpool office, and submit the completed form to the Motorpool office in Thorson Maintenance Center. The driver can also complete the report directly in the OMB-Risk Management reporting site here: https://incidentreporting.omb.nd.gov. Instructions for completing the motor vehicle incident report on-line can be found here: Motor Vehicle Case Submission Instructions.
- The reporting of accidents/damages needs to be completed within 24 hours of the incident.
Please call the NDSU Motorpool at 701-231-9619 with the vehicle number that has been involved in a collision or damaged.
