Bringing F-2 Dependents

Important notes about bringing your dependents and when they must be enrolled in the UHCSR insurance plan to have coverage begin upon their arrival.

If you are in F-1 status:

  • You must be in full-time status.
  • Submit the following documents via our Dependents Request form
    • You will be asked to upload some documents to the form including: 1) Copy of marriage certificate (plus a translation in English), 2) copies of dependents' passports, and 3) official financial documentation
  • Submit form to the OIP and wait a week for documents to be reviewed and dependent I-20 to be issued. We will email you when the documents are ready to be picked up.
  • Send the new I-20(s) and financial documents (bank statements, assistantship letters, etc.) to your dependent(s).
  • Dependents should go to Consulate for visa.
  • Current NDSU F-1 students who return home to marry during their program, must first submit/fax a copy of their marriage certificate prior to issuance of dependent I-20.

Bringing J-2 Dependents

Important notes about bringing your dependents and when they must be enrolled in the UHCSR insurance plan to have coverage begin upon their arrival.

If you are in J-1 status:

  • Be in full-time status.
  • Check with sponsor to be sure they approve.
  • Submit the following documents via our Dependent Request Form.
    • You will be asked to upload some documents to the form including: 1) Copy of marriage certificate (plus a translation in English), 2) copies of dependents' passports, and 3) official financial documentation
    • You will also complete a special form on the last page to verify you understand the insurance requirements for all J-2 visa holders
  • Have enough funds for each dependent (your international advisor will provide you with this estimate); funds must be sufficient for your entire program duration.
    • All J-1 students and J-2 dependents must carry the North Dakota University System mandated health insurance plan through UHCSR. Expense estimate provided will include health insurance costs for duration of program for all family members.
  • Submit form to international office and wait for dependent DS-2019 to be completed. OR, if NDSU did not issue your DS-2019, documents must be provided to your sponsor.
  • Send DS-2019, financial documents (bank statements, assistantship letters, etc) to dependents. Dependents should go to Consulate for visa. A dependent may need to submit proof of insurance to Consulate.
