Forms and documentation may be submitted via:

Authorization to Release Information - (DocuSign) - This form can be used to authorize ECSI and the Student Loan Service Center to release information to a third party, such as a parent, spouse, or financial advisor.
Name Change Request Form (DocuSign) - Requests for name changes require a copy of government issued identification, a copy of an updated social security card and legal documentation confirming the reason for the name change.
Forbearance for Economic Hardship (DocuSign) - Use this form to request a forbearance if you are experiencing financial difficulties and cannot afford payments. The forbearance will allow you to defer payments, however you will continue to be charged interest. Note: Please refer to the Perkins Loan specific forms below to see if you may qualify for an interest free deferment with a post deferment grace period.
Nursing or Medical School Loan Deferment-Forbearance for In-School Deferment - You may submit this form to request deferment while you are enrolled in an eligible college or career school.
Medical Residency Deferment-Forbearance Request (DocuSign) - Use this form to request a payment deferment if you are a medical doctor completing a medical residency.
Nurse Faculty Loan Cancellation Request Form (DocuSign) - This form is used to request a cancellation on Nurse Faculty Loans.
RuralMed Program Residency Certification Form (DocuSign) - RuralMed recipients must provide annual certification for their residency program.
RuralMed Program Practice Certification Form (DocuSign) - RuralMed recipients must provide annual certification that their medical practice coincides with the terms of the RuralMed program.
NOYCE Award Verification and Cancellation Request Form (DocuSign) - This form is used by NOYCE award recipients to verify eligibility for award cancellation.
Pert Faculty Loan (PFL) Verification and Cancellation Request Form (DocuSign) - This form is used by PFL recipients to verify their eligibility for loan cancellation.
Perkins Loan Specific Forms
Perkins Cancellation Form (DocuSign) - Submit this form to request postponement (deferment) and/or cancelation on a Perkins Loan. Must submit annually or if changing jobs.
Economic Hardship Deferment Request - Use this form to request a deferment while you are experiencing financial difficulties or serving in the Peace Corps. May request for up to three years.
Unemployment Deferment Request - Use this form if you are unemployed or unable to find full-time employment. May request for up to three year.
In-School Deferment Request - You may submit this form to request deferment while you are enrolled at least half-time at an eligible college or career school.
Military Service and Post-Active Duty Student Deferment Request- You may use this form to request deferment while you are on active duty military service in connection with a war, military operation, or national emergency or if you were on active duty military service in connection with a war, military operation, or national emergency, for the 13 month period following the conclusion of that service, or until you return to college or career school on at least a half-time basis, whichever is earlier.
Rehabilitation Training Program Deferment Request - This form can be used to request deferment while you are enrolled in an approved rehabilitation training program for the disabled.
Cancer Treatment Deferment Request - You may use this form to request deferment while you are receiving cancer treatment and for six months following the conclusion of your treatment.
Graduate Fellowship Deferment Request - You may use this form to request deferment while you are enrolled in an approved graduate fellowship program.
The Student Loan Service Center (SLSC) provides loan servicing for Perkins, Nursing and other institutional loans issued and owned by the eleven state colleges and universities in the North Dakota University System.
