Disquisition Approval Page
Two separate copies of the Disquisition Approval page must be completed. Copy 1 will have your student ID number and the original signatures of your committee and department chair; this copy is kept in the Graduate School files. Copy 1 must be submitted to the Graduate School before you submit your disquisition for review. Do not include Copy 1 in your disquisition.
Only Copy 2 is included in your disquisition right after the title page, and it will have the committee members’ names, department chair’s name, and the date of approval typed in. Copy 2 should never contain hand-signatures or your student ID – it is only meant to be a representation of the original.

Format Guidelines: Approval Page
Do not include Copy 1 of your Disquisition Approval Page in your disquisition. Only include a fully-completed Copy 2 in your document.

Requirements:

Your document will be published and Copy 1 includes personally identifying information; use only Copy 2 in your document.

Issue: Missing Information
The following information must be filled in on your approval page.
- The title of your disquisition. The title should have the same wording as the Title page, the title you set in your document's properties, and the title you've included with your ETD on ProQuest.
- Your name. Your name should have the same wording as the Title page, and should match your name in Campus Connection.
- The name of your degree.
- The names of your committee members. Your supervisory committee must match the names you submitted to the Graduate School. If you have a co-chaired committee, use the co-chair Disquisition Approval page from our forms page, fill it out, and replace any existing approval page in your document.
- The date of approval for your disquisition. Your date of approval is the date at the bottom of your Report of Final Defense, when it was signed by the Dean of the Graduate School.
- The name of your department chair.
Our form can accommodate up to five committee members by default. If you require more lines, you may add them to the existing form (it's a table), or request a new form from the Disquisition Coordinator; contact them via the link at the bottom of this page.
